The most recent annual sanitation inspection was completed in May 2025 and is past due.

The director was lacking additional training hour topics for the following topics: communicable disease including immunization training, building and premise safety and storing and disposing of hazardous materials.

The annual health inspection was last conducted more than 12 months prior.

Transportation training within the last 12 months is needed for the director.

The training certificates for the director was not available for review at inspection.

Based on the evidence obtained during the investigation, a caregiver left a group of children she was solely responsible for unsupervised to complete additional caregiver duties outside of the classroom.

Based on the evidence obtained during the DFPS investigation, if was found that abuse occurred in the operation.

Based on the evidence obtained during the investigation, it was found that a caregiver hit a child with some form of instrument causing bruising.

Two food allergy plans were reviewed at inspection that was lacking a medical professional signature. One additionally was lacking information regarding symptoms.

One classroom was observed to have one caregiver with 9 children during nap time. The specified age group of the children in care was 12-17 months which is a limit of 5 children of that age group to one caregiver.

Multiple children were observed having their diapers changed and the children did not have their hands washed after. Staff additionally did not wash their hands. Staff used hand sanitizer instead when a hand washing sink was readily available.

Three playground areas that require loose fill did not have adequate amounts of loose fill within the entire use zone. Some areas had less than 2 inches where it requires 6 inches of loose fill. The slide exits, under the swings and near steps is where the loose fill is lacking the most.

An injury occurred at the operation on 6/15/22 that required medical treatment. A report by the operation was not made to child care regulation within 2 days. The incident report signed by the parent and director on 6/15/22 stated that the child received medical treatment on 6/15/22. This was corrected at inspection with an employee of the operation making a self report to child care regulation on 6/20/22.
