8 out of 16 children's infant feeding sheets had not been updated within the last 30 days.

Information obtained throughout the investigation shows that the caregiver did not share relevant information with the incoming caregiver, including parent instructions and details about the child. As a result, continuity of care was not maintained.

Information obtained throughout the investigation shows that a caregiver left a child alone in a classroom. The assigned caregiver left the area without verifying the child?s whereabouts, resulting in a lapse in supervision.

Information obtained throughout the investigation shows that the caregiver did not ensure she knew which children she was responsible for. As a result, a child was left behind in the classroom.

A caregiver was out of ratio, when she was observed caring for 7 children instead of 5.

Information obtained throughout the investigation shows the operation did not report an occurrence that placed a child at risk for injury or harm as required.

Information obtained throughout the investigation shows the caregivers reported they used physical force to make a child nap or rest during naptime.

Observed the Sit-Up Support Ring being used by an 5 month old infant as a tummy time support pillow. This was corrected when the caregiver removed the Sit-Up support ring from the infant's chest.

Upon arrival, observed two caregivers in one classroom with 16 children; SAG - 17 months. This was corrected when the second caregiver in one of the groups arrived to work.

During the follow up inspection, it was found that the teachers were signing and dating the infant feeding instructions.

The toddlers' hands were not washed before eating their breakfast/snack. Prior to mealtime, they were observed playing with toys in the classroom.

One caregiver did not know how many children she was supervising. This was corrected when the caregiver counted the children and corrected her initial answer.

An active background check was unavailable to review for one caregiver who was present and supervising children. This was corrected when the caregiver left the operation.

In the toddler room, observed feeding bibs in the handwashing sink. This was correceted when the bibs were removed from the sink.

Observed three toddlers walking around the classroom with their training cups. This was corrected when the director sat each child down in a chair.

There was standing water and debirs inside of the sensory table located on the playground. This was corrected when the director dump the water out of the sensory table.

Observed children in one classroom sitting at the table without an activity. This was corrected when the caregiver let the children play in centers.

The operation failed to provide care consistent with the child?s needs when a child was served food that he was not supposed to consume on two separate dates while in care at the operation.

One caregiver did not know how many children she was responsible for during inspection.

Good judgement-When a child was exhibiting behavior issues in the class, administration did not provide the caregivers additional resources or support to help the child and protect the safety of the other children in the classroom.

The operation was found during an investigation to be serving candy, soda, and cupcakes to a child in care regularly which does meet the meals and snacks must follow the meal patterns established by the U.S. Department of Agriculture (USDA) Child and Adult Care Food Program (CACFP) that is administered by the Texas Department of Agriculture.

There was no incident report written for an incident where a child was unsupervised at the operation or for behavioral issues with a child that occurred at the operation.

It was found during an investigation that children are using tablets which is not part of the activity plan.

A child was not provided nap or rest time even though the child was in care more than 5 hours.

Multiple children were found during the investigation to have long waiting periods between activities and/or prolonged periods during which children stand or sit specifically at the front desk and/or in the director?s office.

The operation was found during an investigation to be out of ratio in one classroom when they had one caregiver with 18-20 children in a classroom that is only supposed to have 15 children with one caregiver.

One child was unsupervised while present at the operation when the child was able to leave their classroom without the assigned caregiver being aware of their whereabouts in the operation.

It was found during an investigation that there was one incident where a child was unsupervised at the operation that was not reported to CCR.

One staff raised their voice while disciplining the child humiliating the child in the classroom in front of the other children and staff, blaming the child for being hit by another child while present at the operation. Specifically, using a harsh tone while telling the child it was their fault they were hit for following the other child.

The operation does not have a written operational policy that addresses suspension and expulsion.

One child was found to be using a personal cellular phone and/or tablet throughout the day that exceeds one hour per day.

The operation did not share a copy of an incident report with a parent or obtain a parent's signature on it within 48 hours of when the incident occurred.

During an investigation it was discovered the operation was not following the conditions placed on a background check.

During an investigation it was found that children are not being properly supervised when using the Make-Believe Room.

The operation failed to properly notify CCR of an injury to a child that required medical treatment.

The operation provided CCR with a falsified training certificate from Shade Tree Learning.

The operation did not have a sanitation inspection completed in the last 12 months.

Two caregivers in two classrooms did not know how many children they were responsible for. This was corrected at inspection when the caregivers were made aware of the correct number of children present in each classroom.

Two infant cribs did not have snug fitting sheets on the mattress. This was corrected at inspection when the caregivers changed the sheets.

The operation was not conducting monthly fire extinguisher inspections. This was corrected at inspection when the director conducted the monthly check of the fire extinguishers.

The operation did not have a fire inspection completed within the last 12 months.

A child was left on the playground for an unknown amount of time.

Based on the information obtained, it was determined that an infant was observed sleeping on the floor on top of a boppy pillow.

A child was left on the playground for approx. ten minutes.

Caregiver with a pending background check was observed supervising children in care. The caregiver was asked to leave the operation during the inspection.

A caregiver supervising children in care did not have a complete background check on file. This was corrected when the caregiver left the operation.

Observed one caregiver supervising 10 children, ages 13 to 18 mos, during naptime. This was corrected when the second caregiver entered the classroom shortly after CCR.

Hearing and vision screening was not available for review for three of the students records reviewed.

Current infant feeding instructions were not available for review for ten of the 18 infant records reviewed

Per the investigation, it was deteremined that a child was left unattended while in care.
