In room 2 the children were observed receiving and then eating snack before washing their hands.

In room 1 the baseboard of the wall trim was observed to be peeling off the wall which was accessible to children. In room 4 the trash for diapers next to the children toilets was observed to be overflowing with soiled diapers causing a health concern. This was accessible to the children.

The fire inspection was expired for the 2025 year.

One child was left outside unsupervised for approximately one minute and thirty seconds.

A wooden play structure has a bottom palette covered with torn, ripped mesh material which creates a tripping hazard for children. The wooden palette also has gaps between the wood planks which presents a safety hazard.

Two infants under 12 mos old did not have updated feeding instructions on file at time of inspection.

A wooden palette, that was accessible to children in care during outside play time, fell over causing an injury to a child.

One child was left alone and unsupervised outside on the playground area for approximately three minutes when the rest of the group transitioned back inside the building.

The operations infant daily report lacked completed information on the infants summary of activities and infant's overall mood.

The operation lacked written feeding instructions for all infants in care.

One employee was present at the operation and in the presence of children, without having an eligible employment status. The employee was pending fingerprint results from 12/19/22.

The outdoor play area had a fence with growing foliage that included small and large colored berries. Some of the foliage had green round balls that looked like they had soft spikes or points. There was also an orange electrical cord found in the ground. The inspector also observed oyster shells, pieces of wood, broken rocks or cement, and ant piles on the playground.

The operational policy on file was missing the following information: there was no information on the days and months of operation,TB testing, animals at the facility, vaccine preventable diseases for employees, and maintaining and administering unassigned eppi-pens. The information on the promotion of indoors/outdoors activity was also not completed.

The emergency preparedness plan on file did not have a physical address in case of an evacuation and did not discuss how children with limited mobility or who are under 24 months will be evacuated.
