Supervision was not ensured when two caregivers left sixteen children unsupervised for a minute and twenty-eight seconds.

The caregivers did not make good judgment when two caregivers did not communicate with each other, leading to 15 children being left unsupervised.

In one classroom, adult-size scissors, staple removers, and a shipping tape dispenser was observed in an unsecured drawer that was accessible to children. NOTE: This was corrected at time of inspection when the caregiver removed all items from drawer and placed the items out of reach of children.

Based on the information provided during the course of the investigation, one staff failed to use good judgment when she did not count children before leaving the classroom during a transitioning period.

The First Aid Kit was missing the following: Scissors, , Cotton Balls and Thermometer. This was corrected when the Assistant Director placed the missing items in the First Aid Kit. Corrected during the inspection.

Based on the information gathered, a school-age child was left napping in a room and unsupervised for approximately 3-4 minutes.
