One seatbelt on the second row of van #1 was observed broken.

One of the ten children were missing doses of vaccines.

At the time of inspection, no emergency medical information was available for review in the binder.

A written plan to handle transportation emergencies was unavailable for review.

A child's enrollment paperwork did not have a signed medical authorization on file.

During naptime, a toddler caregiver stated there were 12 children in care, when 13 were counted. A Pre K caregiver stated there were 15 children in care when14 were counted.

During an investigation, it was found that a child had a food allergy plan posted in the kitchen that was not signed by a health care professional.

A caregiver did not know how many children she was supervising and gave inspector the incorrect number of children in the classroom. As inspector counted the children, caregver began writing their names on attendance sheet. She did not have one written up for the day.

During the investigation, it was determined that the operation failed to notify CCR of a child injury that received medical treatment within the required timeframe.

Children's product certification was last certified on 1. 3.2021. NOTE: This was corrected at inspection when operation subscribed and certified on recall items.

Fire drills for April were not documented at the time of inspection. NOTE: This was documented was corrected at inspection.

Two of the eight children did not have current immunization records.

Newly hired staff had direct access to children in care before receiving a background check determination of Eligibility
