Children's Lighthouse at Cibolo Canyon

Based off of the DFPS investigation, it was determined that the new caregiver was not informed that the school-age children were not to use blankets outside of naptime by the tenured caregivers.

Based off of the DFPS investigation, it was that two caregivers were aware that the school-age children were not to use blankets outside of naptime but did not remove the blankets when they saw them in use.

A toilet in the toddler classroom had duct tape covering the tank in place of a tank cover and a tank cover of a toilet in a shared bathroom used by preschool ages children was cracked with duct tape placed over the cover.

The director did not have current transportation safety training. Training was last completed on 8/1/2024.

Cleaning supplies were stored under cabinet sinks accessible to children in five classrooms. Note: This was corrected at inspection when staff removed the cleaning supplies from the cabinets in the classrooms.

Four of ten employee records reviewed did not have notarized affidavits on file.

Three of ten staff records reviewed did not have annual training for the previous training year in the required topics of prevention, recognition, and reporting of child maltreatment.

During the course of the investigation, it was determined that caregivers failed to follow transition protocol which resulted in a child being left behind in the classroom and the caregivers not knowing how many children they took outside

A infant bottle in room one was not labeled with their first name and last initial. The bottle was labeled and identified by the caregivers and the assistant director during the inspection.

Two children with diagnosed food allergies did not have a allergy plan signed by both the physician and the parent available for review.

Three school-age children did not have parents and emergency contacts in the vehicle when being transported from their school to the operation. Was corrected at inspection with the contacts were added to the transportation binders.

Caregiver failed to use good judgement when she failed to properly read the name labeled on a child's bottle prior to feeding a child. As a result, a child was fed the wrong bottle.

An infant in room #2 KS did not have the monthly feeding sheet available for review.

A child was reported to have left the transition line and was found in an empty room by a parent. Staff was unaware of the child's whereabouts.

Two children left their class and were able to walk up to the front lobby area, where one child exited the center unattended. Note: the caregiver is no longer employed at this center

The loose-fill in three playgrounds did not have the required depth of loose-fill available in the use zone. The loose fill was less than 1 inch in some areas.

An infant was found sleeping in a crib with a blanket. Note: Blanket was removed immediately.

One of 10 caregivers' files reviewed had 11.25 of 24 required training hours; caregiver was missing some of the required topics trainings.
