Based on information gathered during the investigation, caregivers failed to properly supervise a child when the child was able to wander outside the daycare alone.

Based on information gathered during this investigation, the operation did not provide the parent of the child who wandered outside unsupervised with an incident report.

Based on information gathered during this investigation, the operation did not report to Child Care Regulation that a child wandered away from the daycare unsupervised.

During an investigation, it was determined that the caregivers did not know how many children were in their care on the day of the incident.

This standard was found deficient as part of a DFPS investigation.

During an investigation, it was determined that a child was left unsupervised on the playground for approximately three hours.

During the inspection, it was learned that the operation did not notify Child Care Regulation of the change in director. Note: This was corrected at inspection when the director provided me with their qualifications.

The Director does not have the required training in the following topics: Preventing and responding to emergencies due to food or an allergic reaction; and Administering medication, if applicable, including compliance with ?746.3803 of this title (relating to What authorization must I obtain before administering a medication to a child in my care?)

The primary caregiver in the toddler classroom has provisional conditions and was caring for the children alone. Note: This was corrected when another caregiver was added in the classroom.

One caregiver that provides care for children younger than 24 months of age, does not have the required training. One clock hour of the annual training hours must cover the following topics: (1) Recognizing and preventing shaken baby syndrome and abusive head trauma; (2) Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); and (3) Understanding early childhood brain development.

The operation allowed a 5 year old child in care before requesting that Child Care Regulation amend their permit to add ages 5 years and up. The operation is currently permitted to care for only children between 0-4 years.
