During the investigation, it was confirmed that a child ran out of the back door of the classroom that led to the playground, leaving a parent in the classroom with the remaining children.

The director and a caregiver did not have current training in SIDS, Shaken Baby Syndrome, and Early Childhood Brain Development.

6 out of the 8 children's files reviewed did not have current immunization records.

7 out of the 8 children's files reviewed did not have a signed health statement from a physician on file although children had been enrolled for 12 months or longer.

One caregiver did not have a notarized affidavit of employment on file.

The operation failed to submit the number of employees that have left their employment in 2023 by January 15th.

Loose-fill on playground was observed and measured less than 6 inches in depth.

Based on the information gathered, the operation did not report to Child Care Regulation an allegation that a child was hit by a caregiver.

A caregiver did not treat a child with respect when they arrived late for breakfast. The caregiver stated that the child should help pick up since they weren't on time for breakfast.
