The director has not completed the two required annual training hours on transportation safety for the training year.

A recent gas inspection was not conducted within the past two years. The last document gas inspection was on 8/9/23.

The caregiver that transports children did not have current transportation safety training.

Two out of the three staff files reviewed during inspection did not include the following trainings: Handling, storing, and disposing of hazardous materials and Preventing and responding to emergencies due to food or an allergic reaction.

A stationary school bus structure in the playground was not properly anchored as the bolts were uprooting from the base. Note: Bolts were hammered in during inspection.

Dust was observed accumulating by an air vent. Empty spider webs were observed on the ceiling in the children's restroom and by the door way in the infant classroom. Toys in the school age classroom were observed with a layer of dust. Note: Vents, toys were cleaned and spider webs were removed during inspection.

The infant changing table was moved under a large wall shelf placing infants at risk for injury. This was corrected at inspection.

It was found that children in care have been allowed to go from one classroom to another without supervision.

A caregiver failed to interact in a positive manner when she tried to pick up an infant child by her underdeveloped arm.

The operational policies did not contain the indoor/outdoor and abuse and neglect policy.

Two out of the eight records reviewed during inspection did not include proof of education.

One out of eight records reviewed did not include orientation.

There were no cribs observed for two non-walking infants in the infant classroom. Note: This was corrected at inspection as the Director brought in two cribs into the infant classroom.
