At time of inspection a toddler caregiver stated to have 6 children when the class had 7.

At time of inspection the toddler playground unitary surface was observed to have multiple cracks and holes. The grass in th toddler playground was observed to eb approximately 3-4 inches tall.

Written feeding instructions had not been updated for two infants since November 2025.

Four of ten staff records reviewed revealed there was no documentation of pediatric first aid being completed by four caregivers prior to them caring for children unsupervised.

Two of two director files reviewed for annual training during inspection did not include training in required topics of administering medication, preventing and responding to emergencies due to food or allergic reactions, and handling, storing, and disposing of hazardous materials.

At time of inspection, a child's medication was past its expiration date.

Three out of ten children files did not have current immunization records in their records.

A caregiver had direct access to six children in the toddler classroom prior to a background check being submitted. Note: This was corrected at inspection when the caregiver was sent home.

A Pre Kinder caregiver reported she had five children in her care when there were six children in her care.

At time of inspection a toddler caregiver stated to have fourteen children when the class had fifteen. At time of inspection a toddler caregiver stated to have ten children when they had nine children in the class.

At time of inspection, seven out of the ten children files reviewed did not have an admission date on file.

At time of inspection, operation had two incomplete food allergy action plans for children.

At time of inspection operations last health inspection was conducted on 3.21.24.

At time of inspection, four out of ten children files did not have a health care statement.

At time of inspection, a childs medication was past its expiration date.

At time of inspection, a childs food allergy action plan was not signed by parent nor health professional.

The operation has not obtained an updated fire inspection.

Two maintenance staff were present at the operation and access to children without a eligible background check.

An 11-month-old child as observed sleeping in the crib while a pacifier strap was in the crib.

The operation fire extinguishers service tags read February 2023. The fire extinguishers have yet to be replaced. The operation has a quote dated 5/20/24 for fire extinguisher replacements but have not acted on it.

The opertion's fire inspection was on 3/27/24. The staff was not aware that the fire extinguishers needed to be replaced and that the fire section of the reports was marked as failed.

The operation has outstanding fees going back to December 2023, March 2024 and the annual fee since 3/17/24.

There was no recorded fire drill for the month of April 2024.

The operation did not have a current fire inspection available for review.

A caregiver was seen interacting with a child in a manner that was inappropriate. The caregiver picked up a child from the floor where he was laying down up to her waist and held him by his upper arm and neck area.

A caregiver was seen on live footage roughly shuffling a child's legs while he was on his nap mat, restraining his legs by placing her leg on top of his, and patting his back in a hard manner.

The operation failed to report the number of employees that left their employment in 2023 by the required January 15th deadline.

During the course of a DFPS investigation a caregiver was observed on video pulling a foam chair out from under a child who was sitting on it, causing her to slide to the floor.

Staff training records were not available to review during inspection.

The blue slide on the school age playground has an crack at the end of the slide where the children exit.

During the investigation, it was determined that a positive case of covid was not reported to Licensing.
