During a DFPS investigation, video shows a staff intentionally pushing a child forward causing the child to hit their face with a organizer cart that was in close proximity resulting in an injury.

This standard was found deficient as part of a DFPS Investigation.

The caregiver failed to use good judgement when they had a child go to another classroom to ask for assistance with a child injury. The caregiver also failed to record the incident accurately on an incident report form.

During a DFPS investigation, it was determined the caregiver picked up a child by their arm and forcefully walked them to another area in the room.

At time of inspection a PreK caregiver stated the class had thirteen children when the class had twelve.

At time of inspection, six infants sitting in restrictive devices were not strapped in. This was corrected at inspection.

Parents of children who were involved in a non-routine situation that placed them at risk were not notified until ten days after the incident.

Three out of the ten records did not have current health care provider statements.

The operation had five infants with a specified age group of 0-11months old with one caregiver. *The operation brought in another caregiver immediately.

Carbon monoxide detectors were not documented as being checked the previous month.

A child with medication kept at the operation did not have the original prescription boxes available.

A caregiver in the infant room was in direct care of the children in the classroom without an eligibility letter on file. NOTE: The caregiver was sent home during the inspection.

Six of the ten children were missing Healthcare statements. Four of the ten children records were missing Emergency addresses. Four of the ten children files were missing Authorization emergency hospital. Two of the ten children files were missing healthcare physicians information.

The operational policy did not specify a written policy for promoting physical activity.

One of the ten children files was missing current immunization records.

Asthma medication was currently being stored in the closet which expired 4/2022.

One caregiver was observed in the toddler classroom with 12 children (specified age of 2 years) resulting in being over 1 child.

In the toddler classroom the shelves were observed bare. NOTE: This was corrected when the items observed above the shelf were brought down into the shelves.

A two month old infant was observed awake on a blanket in a play yard.
