A play structure on the playground was observed to have the rail road ties that were anchored on one side sticking up and were not flush with the metal beam and the other side did not have the anchors at all. Noted: the anchors that were present were hammered down to be flush with the beam.

A child was observed to be sleeping in front of an exterior exit door. Noted: this was corrected during the inspection when the child's mat was moved away from the door.

The licensing Parent Notification Poster, was not posted. Noted: this was corrected when the poster was provided to the operation and was printed and posted on the parent bulletin board in the lobby area.

The reviewed operational policies were found to be missing items, such as, Suspension and expulsion of children, Safe sleep policy for infants from birth through 12 months old that is consistent with the rules in subchapter H, Promotion of indoor and outdoor physical activity that is consistent with Subchapter F, Parent rights that are consistent with the rules in Division 5, Procedures for parents to review and discuss with the child-care center director any questions or concerns about the policies and procedures of the child-care center, Instructions on how a parent may access the Texas Abuse and Neglect Hotline, information regarding Preventing and responding to abuse and neglect of children, including: Methods for increasing employee and parent awareness of issues regarding child abuse and neglect, including warning signs that a child may be a victim of abuse or neglect and factors indicating a child is at risk for abuse or neglect, (C) Methods for increasing employee and parent awareness of prevention techniques for child abuse and neglect, and (D) Strategies for coordination between the center and appropriate community organizations, Information on vaccine-preventable diseases for employees, Procedures for supporting inclusive services to children with special care needs. The policy must address the requirements outlined in ?746.2202, information regarding area being a gang-free zone.

Current activity plans were not posted in two of the three classrooms. Noted: this was corrected during the inspection when the current lesson plans were posted.

A slide structure with a platform of 27 inches was observed to not have the required six inches of loose-fill installed around the structure.

The director did not have a current First Aid and CPR certification available for review. The certificate that was available was expired as of July 29, 2025.

Three fire extinguisher's tags indicated that they had last been serviced August 2024.

One caregiver, who has been employed over 90 days, did not have current First Aid Certification.

There was not a current Form 2885 signed and avilalble for review. Noted: This was corrected during the inspection when the CPSC website was reviewed and form 2885 was signed and posted.

Per the health and safety audit, the director's training was reviewed and there was no training certificate available for review in the required topic: Medication Administration. Noted: this was corrected during inspection when the Medication Administration training for the current training year was used to cover the missing training in the evaluated year.

One of the six children's files did not have a parent or physician siged health statement availalbe for review.

An infant was sitting in a highchair without safety straps fastened.

An infant under 12 months was observed to be sleeping in a crib with a blanket. Noted: The blanket was removed during the inspection.

Recite: A caregiver was observed using her cell phone while caring for two children.

One caregiver present did not have her notarized employment affidavit.

There was food stored in the refrigerator uncovered. Noted: This was corrected during inspection when the food was disposed of.

A caregiver was observed seated using her cell phone while the group was outside on the playground.

One staff file reviewed did not have a copy of their photo ID in the file.

It was determined that an individual was present at the operation with children that did not have an active background check.

One caregiver present did not have her notarized employment affidavit.

One caregiver did not have required training in food allergies and Communicable diseases. *A training certificate in communicable diseases from the current training year was added to the previous training year to cover the topic.

One caregiver did not have the required 24 hours of annual training. The caregiver had 22 hours of training. *A training certificate for two hours in communicable diseases from the current training year was added to make up for the missing 2 hours.

In the pre-k classroom there was water conditioner liquid and cleaning supplies that were accessible to children.

Furniture in the toddler age room (18-24) was observed to not be age appropriate for the children.

During the investigation process it was determined that lunch is being served passed three hours from the first meal/snack being served.

Two individuals were able to be counted in ratio before receiving background check results.

An infant age child (0-17 months) was being combined with children ranging in age from 2-5 years of age.

An individual who has a background check of a volunteer was observed to be in a classroom alone with children. Noted: this was corrected during inspection when the volunteer was removed from the room.

An infant was observed to be laid down into a crib with an additional mattress added. Noted: this was corrected during the inspection when the second mattress was removed from the crib.

Two infant aged children were left in a room unattended.

The fence sourrounding the outdoor play area was observed to have areas that a child could push through to get to the other side.

The operation has not been completing a monthing inspections of the building and grounds.

The operation did not have a current health inspection.

The emergency prepardenss plan did not include information on how care will continue in the even of an evacuation/relocation.

The fence on the outdoor space was observed to have holes and metal extending out posing a risk of entrapment and puncturing.

The drills did not include necasssary information such as the time of the drill and the length of evacuation.

A person present at the operation did not have an active background check.

An incident report was not written for an incident in which a child was able to leave the operation for a minimum of 20 minutes.

Neglectful supervision occurred when a child was able get 3/4 of a mile away from the operation unnoticed for a minimum of 20 minutes. The child was found by a passerby unharmed.

Supervision was compromised when a child was able to leave the operation's premises unnoticed.

It was determined in the investigation process that an individual with a volunteer background check clearance, was left unsupervised with a group of children.

Through the investigation process it was determined that the person left in charge, in the director's absence, was not qualified as she did not have a current and clear background check.

During diapering, a caregiver was observed to not wash the child's hands.

During diapering, a caregiver was observed to not change gloves before initiating a diaper change of another child. Noted: This was corrected during inspection when the caregiver removed her gloves and washed her hands before the next change.

A caregiver was observed utilizing the handwashing sink to rinse bottles. Noted: This was corrected during inspection when the caregiver was given information on why the handwashing sink cannot be used for cleaning bottles.

Caregiver stated that children age 12m- 2 years, who are all walking, are sleeping in cribs.

The cribs were observed to not have labels or a system to identify the crib to the child that uses that crib.

Activity plans for the month were not posted.

A child was left alone in a classroom, when the caregiver left the room to open the door to allow inspectors in to the building.

Three of the eight children's files viewed did not include the required phone number or address for the emergency contact.

The current employee list was not posted. Noted: This was corrected when a list was typed and posted on the parent board.

Spray bottles were observed to be on the changing table accessible to children. In the children's bathroom, a cabinet was not utilizing the installed safety locks, leaving the cleaning supplies stored in cabinet to be accessible to children. Noted: this was made complaint during inspection when the cabinet was locked and the spray bottles were moved to a higher cabinet.

The bottles and cups observed were labeled with only the child's first name or not labeled at all. Did not include the required first name and last initial.

A current Fire inspection was unable to be provided for review.

The classrooms being utilized were changed to accommodate different age groups before notifying licensing.

Two of the eight files observed did not include a health care provider statement, past the 12 months after the enrollment date.

There were items in the first aid kit that were missing. The items that were missing were: Thermometer Gloves

There were two caregivers at the operation that did not have their CPR and First Aid certification available for review.

The television in the Preschool room is not anchored to the shelf that it is set up. The operation corrected this at inspection when the television was removed form the classroom.
