At time of inspection, multiple infant bottles were observed to not be labeled. NOTE: This was corrected at inspection.

At time of inspection, infant monthly feeding instructions were not updated.

During the course of the investigation, it was noted a toddler was left unsupervised for one minute in a classroom.

During the investigation, it was noted that a operational staff did not exercise good judgment by allowing a toddler to play in the playground dressed only in a top and underwear.

During diaper changing the child didn't wash hands after. NOTE: This was corrected at inspection when the child washed hands.

At the time of the inspection, a caregiver was observed supervising children with a pending backgrouns check. NOTE: This was corrected when the caregiver left the facility.

At the time of diaper changing and sanitizing the mat, the caregiver didn't wash their hands after removing their gloves. NOTE: Corrrected at inspection when the caregiver washed hands.

A pre-k room did not have outlet covers over two outlets. *This was corrected at inspection*

The operation did not have the smoke detector documented as tested for the previous month.

The operation did not have a current employee list at inspection.

During the walkthrough, a caregiver did not know the ages of the children in her class.

During the inspection, there was a bottle of Windex in an unlocked cabinet that was accessible to children. There is also a metal cabinet outside that the children accessed. THe cabinet contains rusty nails, spray paint, and other hazards.

During the inspection, it was observed that a garbage can that contained food did not have a lid on it. This was accessible to children and a toddler was observed with their hands in the garbage.

During the walkthrough, a caregiver was heard yelling at children. The caregiver was inside the building and was heard from outside the building.

During the investigation, it was determined that a caregiver demonstrated poor judgement by allowing a child to sleep on top of a picnic table for an hour while outside on the playground.

During the investigation, it was determined that a caregiver used prohibited discipline when caregiver grabbed a child by their arm and wrist and attempted to pull the child up from the floor.

At time of inspection, school aged caregiver stated class had ten children when class when there were eleven. - This was corrected during the inspection.

At the time of review the operation did not have a documented smoke detector test completed for the past month.

Six out of the seven staff files reviewed during inspection did not include the pre employment affidavit. Note: This was corrected at inspection when the caregivers completed the form and added to the file.

The operation has not received a health inspection.

The operation has been providing care regularly for compensation without a permit from licensing.

The use zone for the slide overlapped with another playground equipment's use zone.

Construction materials and debris were observed accessible in the playground space. A six inch metal stake nail was observed protruding from the barrier that surrounds the playground equpriments use zone.

A caregiver with a pending background check was observed caring for children during the inspection.
