There was one caregiver with six infants. The ages of the infants were 6 months, 10 months, 2;15 months, and 2;18 months old. The operation was over ratio by 1 infant.

Four of the eight staff files reviewed did not contain training topics on Emergency Practices for their annual year.

The infant rooms were located in the cafeteria due to classroom maintenance. The space was not pre-approved by licensing for use in that room for infants or any other classroom in the center. **This was corrected at inspection when the infant rooms were relocated from the cafeteria to other unused classrooms that were adapted to meet the infants' needs.

During the investigation, it was found that the caregiver did not practice proper supervision of all the children in her care after leaving the room with the injured child, resulting in a child being left unattended.

During the investigation, it was determined that the child was not released to the person on the pickup list, and no court order paperwork was on file.

While reviewing the files, one child was missing an emergency contact person other than a parent.

Two of ten staff records reviewed were missing training for the previous training year in the required topics of; 1) Emergency preparedness 2) Preventing and controlling the spread of communicable diseases, including immunizations 3) Administering medication 4) Preventing and responding to emergencies due to food or an allergic reaction 5) Understanding building and physical premises safety and 6) Handling, storing, and disposing of hazardous materials.

One of ten staff records reviewed were missing 24 hours of annual training for the previous training year.

Two containers of liquid carpet cleaner were stored under the hand washing sink unsecured in the Pre-K classroom. Note: This was corrected at inspection when the director removed the cleaning products from the classroom.

Bottles stored in the infant refrigerator did not have child's name labeled on the bottles.

Diagnosed food allergies for one of the children enrolled were not posted at the operation. Note: This was corrected at inspection when the operation created an updated list showing all children with diagnosed food allergies.

Epi pen on file for a child was expired as of 12/2020.

2 out of the 10 children's files reviewed were missing updated immunization records. Note: Records were updated for 1 of the 2 children during the inspection.

Gas inspection at the operation was past due. Current gas inspection was due 06/2022 and the last inspection was conducted 06/2020

There was no emergency evacuation and relocation diagram posted in either of the two school age rooms. Note: This was corrected at inspection when the operation posted the diagrams in each room.

Information gathered during investigation determine an incident occurred with three children on the playground structure. Note, there were two staff outside with the group, but not properly monitoring the children.

Operation did not share an incident report with the parents of three children regarding a situation on the playground.

The operation failed to report to Child Care Regulation an incident on the playground with three children.

Two infants were observed sleeping on a baby bouncer.
