During the inspection the rail road ties or wooden border on the playground was broken with sharp points on several sides.

An infant younger than 12 months was sleeping on a boppy pillow while in their crib. The child also had a blanket, was wearing a pacifier tied to a lanyard, and was wearing a bib. This was corrected when the items were removed from the crib and the child.

A caregiver left her room to answer the front door. This was corrected when she immediately went back to her room.

One of six children's files did not have an updated immunization record.

The permit holder/owner did not have verification of their annual training completed.

A toilet brush was found in the restroom used by toddlers and pre K aged children. NOTE: This was corrected when the permit holder removed the toilet brush from the restroom.

A child was enrolled and began attending the operation without a person to contact in case of an emergency. Child is no longer enrolled.

A child was enrolled and began attending the operation with the special needs question not answered. School aged child did not have full name/address/phone number for child's school. Child is no longer enrolled.

A new employee has had unsupervised access to children and is not certified in First Aid.

One of the two transportation vehicles had three booster seats that were expired. They were all removed from the vehicle and will be replaced.

The pre k class was taken outdoors with no supply of drinking water on hand for the children.

It was determined that two caregivers admitted to seeing marks on a child but did not report such findings as required.

During inspection the transportation van purchased recently is not equipped with a child electronic safety alarm.

During inspection, the outdoor playground was observed with less than the required depth of 6 inches of mulch.

One of four employee files reviewed did not have verification of annual training completed. J.G. had verification of only 16 of required 24 hours completed of annual training.

One of four children's files reviewed did not have medical authorization portion of enrollment forms filled out. This child's file did not have any healthcare provider information.

One of four children's files reviewed did not have a health statement for child from a physician.

During inspection, it was observed that the fire inspection is overdue, last one conducted in Jan/22021.

Four year old child was injured and parents were not notified within the 48 hour timeframe.

A combined classroom was with eleven children had a eighteen month difference between the youngest child and oldest child. The age of the youngest was twenty months and the age of the oldest was five years old.

Plastic window in the playscape was broken and opening was big enough where children put the hand through. The broken part of the window had sharp edges that could have potentionally harm a child. This was corrected at inspeciton when director removed the broken plastic window from the playscape.
