During the course of the investigation, it was determined that a caregiver was on her phone while driving.

Based on the evidence gathered, a two-year-old child was allowed to play on a fulcrum seesaw while in care, which resulted in a minor injury. This was corrected at inspection when the fulcrum seesaw was removed from the playground and made inaccessible to children younger than 5 years of age.

Based on the evidence gathered, at least three children who require a child safety seat or booster seat while being transported, were not secured in the proper safety restraint device during the vehicle accident.

Based on the evidence gathered, the operation did not immediately notify parents that their children were involved in a vehicle accident. The operation waited to inform parents at pick up later that day. One parent wasn't notified until the next day.

During this investigation it has been determined that the operation has violated the standard for children 12 years old and under riding in the front seat.

Two electrical cords were hanging over the changing table in reach of infants. This corrected at inspection when caregiver, unplugged the cords out of the reach of infants.
