During the inspection 3 caregivers did not know how many chidlren they had in their care.

A child was observed to not wash their hands after their diaper change was completed. Noted: this was corrected during inspection after the caregiver was reminded to wash the child's hands.

It was determined that corporal punishment was used when a caregiver was observed to grab and throw a child to the ground.

Based on the information gathered, a child was left in the classroom asleep, while the rest of the group transitioned to the playground.

During inspection it was observed that two medications were observed expired. Note: Corrected at inspection when center director removed the medication from the cabinet.

During the course of the investigation, it was determined that a caregiver pulled a child from the arm resulting nursemaid's elbow. It was also determined that the caregiver hit a child on the head with 4 fingers.

During the investigation visit, several caregivers were heard by Child Care Regulation inspector on the two-way radio stating they were out of ratio.

Based on surveillance video, it was observed that caregiver pulled a child by one arm several times. Caregiver ran after child and pulled child up from the back of shirt, child attempted to pull away and caregiver pulled child up from the back of shirt. Caregiver is observed pulling another child out by one arm out of the tunnel in the playground. Caregiver ran after another child and grabbed child by the back of shirt and leading the child by the shirt. Note: Caregiver was terminated.

Based on information gathered, it was determined that caregiver yelled at the children during outdoor time. Note: Caregiver was terminated.

Based on information gathered, the operation did not notify a parent immediately upon discovering surveillance video that showed a caregiver using inappropriate discipline on their child. The footage was found on 11/6/2024 and the parent was not notified until 11/21/2024.

There was a teething bead string attached to a pacifier in the infant classroom, room 3. Note, this was corrected at inspection when staff removed the item from the classroom.

The last fire drill conducted and recorded was in April 2024.

A toddler was left unsupervised on the playground.

A child was left unsupervised in the classroom for thirty minutes.

A child was left unsupervised for approximately three minutes outside.

In the course of this investigation, training on SID?s and Shaken Baby Syndrome was requested for all three caregivers in the infant room. The operation only provided training for one caregiver.

Operation failed to make a self-report within the required timeframe of being informed that the infant received medical attention.

The operation did not notified licensing within the required timeframe after learning an infant received medical care.
