During inspection, director annual trainings were not available for review.

A child with a known egg allergy was served food with egg product in it.

In the two year old classroom there was a changing mat but was a non- absorbent changing mat. In the three year old classroom their was not a changing mate for diaper changing.

A caregiver verbally rejected a child when she told the child to get away and leave her alone. The caregiver physically rejected the child by placing her hand up to keep the child away from her which resulted in the child falling on their bottom.

A child was observed with a spray bottle containing soap. This was corrected at inspection when the bottle was taken away and stored out of reach of children,

In the classroom Hundred Acres, designated to 18 months-36 months the two caregivers verbally said that they had 10 children when their was a total of 12 children present in the class.

The operation health inspection was expired (5/5/22)

Two infant feeding schedules were expired.

Based on the information gathered the parents were not informed when a child fell and was injured but did not seem serious at the time.

Based on the information gathered the incident report was not signed by the parent when a child was injured and required medical treatment.

Based on the information gathered the caregiver in the 12 to 18 month old class was yelling at the children in care.

Based on the information gathered the 12 to 18 month old children were placed in time out for more than two minutes per year of age.

Based on the information gathered one caregiver was grabbing the children roughly and aggressively.

Based on the information gathered, the caregiver in the 12 to 18 month old classroom was slapping the children on the hand when they misbehaved.

Based on the information gathered, the caregiver in the 12 to 18 month old classroom was cursing in front of the children.

6 out of 10 staff files did not have the required annual training pertaining to: Building/premises safety, responding to food allergy emergencies, administering medication, hazardous materials, child abuse and neglect.

Two out of ten children files were missing updated immunizations. Note: One immunization record was corrected at inspection when the parent's sent a current record via email.

A child in the 13-18 months classroom and a child in the 18-24 months old classroom were found using pacifier beads. Note: Director removed beads immediately from the children.

During the course of the investigation it was determined that 2 caregivers neglectfully supervised a three year old child when that child was left on the playground for an hour and forty minutes.

A child was left on the playground unsupervised after the class transitioned back into the classroom. The child was found after an hour and forty minutes later. When found, he was red faced, sweaty and crying.

During the course of the investigation, it was determined that parents were not immediately notified of a situation that placed their child at risk. The parent was notified an hour later by the operation regarding the incident.
