Seven of the ten children's files did not have updated immunization records available for review.

Napping equipment (sleeping bags) were observed to be placed very close together, not allowing a clear walkway for staff or children. A caregiver was observed to have to step over the mats to make her way to another area in the room.

Two of the nine caregiver files reviewed did not have training certificates available for review in required topics such as; SIDS/Shaken baby/Early brain development, hazardous materials, medication administration, and food allergies.

For investigation 3195508, the equipment was removed from the active outdoor play space. Recite: a piece of stationary equipment was observed to be in the outdoor active play space with a missing seat, leaving a protruding metal post sticking out from the equipment.

Caregivers were observed using the hand washing sink in the infant room to clean silicone bibs and trays from the high chairs.

Spaces designated as licensed space for children are being utilized as storage and not set up for children's use.

During the course of the investigation, it was determined that an outdoor toddler climber was being used when it was missing a slide. The result was an unprotected 24-inch drop.

During the course of the investigation, it was determined that caregivers were not positioned around the playground to where they could actively supervise the children in their care.

It was determined in an investigation that good judgment was not exercised when a functioning coffee maker was provided to children to be utilized as a toy.

It was determined through the investigation process that parents and guardians are not being made aware of less serious injuries (bites, bumps, scrapes, etc.) when they pick up their children for the day.
