One out of ten staff files did not have a notarized Affidavit for Applicants for Employment with a Licensed Operation. NOTE: This was corrected at inspection when the staff member provided the notarized document.

Ten out of the ten staff files did not have signed acknowledgment of receiving Operational Policies.

Nine out of the ten staff files reviewed did not have training hours in all required topics from the last full year of employment.

During the inspection, two children's immunization records were not current. Note: administration was able to obtain one record during the inspection

Based on the information gathered, a child was left unsupervised outside for approximately one minute. Note: Prior to inspection; caregiver's were reprimanded.

During the investigation, it was confirmed that a child exited the back door of a classroom to the playground and went unnoticed by caregivers for approximately 2-5 minutes.

During the walkthrough, a caregiver was observed grabbing a child by the arm during a transition.

Two children with medication kept at the operation expired in 11/2024, 1/2025 and 7/2025.

The operation did not contact a child's parent to report a possible injury that required medical intervention after the child reported being hurt and showed signs of a possible injury.

Caregiver failed to share information regarding a child's injury to the closing caregiver.

A caregiver with an inactive background check from 3/2023 had direct access to children in care. NOTE: The caregiver was asked to leave the operaton and a background check was submitted by the opertion.

Two allergy plans on file did not contain the parent signature on the form.

A caregiver failed to conduct a proper name to face count when arriving outside at the playground, therefore a child was left inside unattended.

An inhaler and Epipen did not have the original prescription box for the child in care. A bottle of Benadryl did not have a child's name on the container.

1 out of 10 immunization records reviewed was missing and updated record in the child's file.

3 medications had expiration dates of 9/22, 5/23 and 5/22.

There were 2 children left outside the classroom unsupervised for 1 to 3 minutes.

There was a child that was left inside the classroom for 1 minute before being regrouped with their class.

It was determined that there was a child that was left in a classroom unattended for 30 seconds.

There was a child left unattended outside for 1 minute before taken inside and returned to the classroom.
