During the inspection, the director did not hane any annual trainings. - Trainer Marlene Braserio (TRS) has not provded the trainings the operation paid for. An extion was granted.

The infant caregiver did not have any annual trainings completed. - an extention was given at the follow up inspection due to the trainger not providing certificates for the completed trainings.

During the inspection it was found the nap mats were not labeled with child's name or number.

A caregiver did not wash a toddlers hands after changing their diaper. A caregiver did not sanitize the diaper changing table before placing another child onto the table.

The fire extinguisher had a last serviced date of June/2022. Verification was not provided that a more recent service was conducted.

Caregiver left three two year old children unsupervised to tend to a toddler in the restroom.

The operation did not submit the required information by the due date of 1/15/24: number of employees that left employment during 2023.

During the course of the DFPS investigation it was determined that operation staff are using physical activities such as push-ups, wall squats, and being forced to run as punishments when children misbehave.

A new hire has been employed as of 4/20/23 and has been in charge of a classroom by herself. She is not certified in First Aid.

It was observed that the gas leak inspection is over due. Last inspection dated 7/2019.

The operation failed to properly verify that all staff employed at the operation were listed on the people list and had a current eligible background check in place; resulting in an employee actively working at operation since 7/17/2018, without a background check.

During investigation conducted by CCI, it was determined that an employee had been working at operation since 2018, there is no record of a background check being run on this employee.
