A caregiver did not know the number of children in their care when asked.

Recite: Active supervision was not practiced when a child left his assigned group to attempt to open the operation door to let the CCR inspector in, when the caregiver left the group to open the door to let the CCR inspector in, and when a child was observed to be stabbing other children with a pencil.

A infant was observed to be seated in a swing that was placed in a crib.

The caregiver in the toddler room was unable to provide the ages of the children in her care.

A child, identified as 18 months, was observed to be watching cartoons on the tablet.

It was determined in an investigation that proper supervision was not utilized when a child was left alone and an unfamiliar person was able to enter the building and classroom spaces and access a child.

Children were observed eating snack on the floor.

Whole grapes were served to children 2-4 years of age.

A fourteen month old infant was in the transition classroom where the oldest child was ten years old.

During an investigation inspection, a ten year old was left supervising a two year old child using the restroom while the caregiver took the remaining children to the school age classroom to lay down for naptime. The children were alone with no supervision for less than one minute.

The health inspection had not been conducted. The last report is dated 5/12/23.

A caregiver was observed not washing a child's hands after changing a diaper. The caregiver did not wash her own hands after diapering a child while wearing gloves. Toddlers resumed class activities without washing their hands after returning to class from the outdoor playground.

A screen time device that was playing a video was placed in front of an infant who was sitting in a restrictive device while caregiver changed another infants diaper.

During a DFPS investigation, a folding table and two loose wooden panels that were leaning on wall fell over and blocked the door to the mens restroom from opening, thus trapping the investigator inside.

One of seven caregivers did not have First Aid certification. Her start date was 7/31/23, she has provided unsupervised care to children.

Two of six files did not have an updated immunization record in the file.

Operation failed to report COVID cases within 48 hours after receiving notification. Operation was notified on 7/21 and 7/22 but reported to CCR on 7/25/22.

Observed caregiver in toddler class viewing her cell phone during snack time.

During investigation it was determined that the caregiver involved in allegation admittedly used threats of write ups instead of alternate positive methods of discipline. Caregiver also admitted to grabbing and holding onto child by the arm and not releasing when child pulled away, instead grip became tighter causing pain.

The new center van is not equipped with an electric child safety alarm.
