Three out of nine files audited were missing required trainings.

In the infant classroom a caregiver did not wash the infants hands after changing the infants diaper. This was corrected at inspection when the caregiver washed the infants hands.

There were two infants observed sitting in the seats at the feeding table without any straps. This was corrected at inspection when the infants were removed from the seats.

The fire extinguisher mount in the van was observed to be broken and was unable to keep the fire extinguisher secure.

A caregiver was observed doing a diaper change and neither the caregiver or the child washed their hands once the change was completed. Noted: this was corrected immediately when the caregiver was reminded to wash the child's hands and their own hands.

During transport, children's emergency medical transport and treatment authorization was not availalbe. noted: corrected during inspection when the admission forms were added to the clipboard.

Based on information gathered, caregiver did not interact with child in a positive manner, when caregiver picked up a child by arm roughly and squeezed a child's hand roughly.

Based on information gathered, it was determined caregiver used inappropriate discipline by grabbing two different children roughly by their arm, pulling child's arm and pinched a child.

A slide on the Playground has 2 screws sticking out, and 1 bolt is missing from the slide. In the school age classroom there is a divit where a chunk of drywall missing.

3 children's' files do not have emergency contact address' listed

2 children were not found in the tracking system that were present. This was corrected when one was added to the system, and one was signed in.

A sandbox filled with rain water was open on the playground while children were present. This was corrected during the inspection when the water was dumped out and the sandbox was covered.

3 Classrooms do not have cubbies for children to store their belongings labeled with their names.

It was found a caregiver placed an infant in a bouncy seat on top of a table.

1 of 7 caregiver files are missing CPR& First aid training.

An outdoor play structure has a damaged piece of wood exposing a pointed screw.

2 infants were in high chairs equipped with safety straps that were not fastened.

2 of 7 files reviewed were missing training in the required areas of SIDS/Shaken baby, Aministering medication, food allergies, and hazardous materials. 1 of 7 files is missing transportation training.

During the course of the investigation, it was determined that the operation was not open to parents of children in care during operating hours.

During the course of the investigation, it was determined that a caregiver used poor judgment and demonstrated a lack of self-control when they raised their voice and used profane language in the presence of children in care.

During the course of the investigation, it was determined that a caregiver used a chair with a strap to restrain a child from moving during timeout.

There were toilet cleaning supplies that were located in a cabinet underneath the sink in the school age classroom. Note: This was quickly corrected at inspection when the cleaning supplies were removed from under neath the sink to a high shelf located on the wall.

There were four children files out of the ten viewed that did not have a health statement signed by the child's health care physician available for review.
