During the course of the investigation, it was determined that the caregiver was unable to provide adequate supervision to prevent children from exhibiting inappropriate behavior.

The operation did not report the number of employees to leave the operation by the required date.

The operation did not have documented that the fire extinguishers were inspected for the past month.

Operation was missing a fire relocation diagram in a classroom. *Corrected at inspection

A caregiver did not have the required trainings in emergency preparedness, preventing and controlling communicable diseases, administering medications, preventing and responding emergencies, building premises safety, and hazardous materials.

The director of the operation admitted to drinking alcohol on the premises after hours in their office. Information gathered during the investigation determined that while the director was not supervising children, children were still in care during this time.

The operations fire inspection expired in September of 2023 and did not have a current inspection available for review.

The last fire inspection conducted was 7/19/21. There is no current fire inspection on file.

Children were allowed to walk alone from the classroom to the designated pick up place and were left unsupervised for several minutes without a staff member present.

There was a classroom that had the lights off for nap time. The children/caregiver could not be seen in the classroom. The operation corrected this at inspection when the caregiver in the classroom opened another door leading to the classroom, this added additional lighting to the classroom. All the children accounted for and seen with the additional lighting.

There was a child that was placed by one of the exit doors of the classroom for nap time. This was corrected at inspection when the caregiver picked up the child's mat and moved him to a different spot in the classroom.
