A caregiver did not intervene while watching children swing the restroom door open and close resulting in a child pinching their fingers with the door.

The caregivers personal bags were on the counter at hands reach of children in the younger two year old classroom. Note, this was corrected at inspection.

Ten crib mattresses in the infant I and II classrooms had the vinyl torn.

The fence line shared between the toddler and Pre-K playground had a gap at the bottom of the chain link fence. The fence line on the Pre-K playground towards the left side of the gate had pointy metals from the chain link fence position upwards and out. The toddler playground had several piles of cement approximately twelve inches high next to the concrete edge surrounding the unitary surface making it a tripping hazard.

Water was observed collected in a water table on the playground. Note: This was corrected at inspection when the water table was tipped over by the Director.

Caregiver was observed asleep while supervising the children in care.

Based on the evidence gathered in the investigation conducted, it can be determined that a caregiver is yelling at the children.

Out of 5 children's records reviewed, 4 records required updated immunizations.

A television in the game room was not anchored.

A step stool in the two year old room had a large crack through the middle creating a safety hazard.

A fire exit in a two year old room was blocked by classroom materials and a backpack. This was CORRECTED AT INSPECTION when the materials and the backpack were moved.

There was an infant left unattended on the diaper changing table while caregiver grabbed the wipes from underneath a cabinet.Caregiver was not at arms reach of the infant.

Five children files were missing current immunizations.

Three different classrooms were observed of having a caregiver's personal bag on top of shelf or chair at hands reach of children. Note: Corrected at inspection

There was a newborn child combined with twenty five other children ages 2 -5 years old in the classroom.

A three month old infant was sleeping in their crib with a pacifier clip with silicone beads over their stomach.

There was one out of the ten children files viewed that did not have their authorization for medical treatment section completed in their enrollment paperwork. This was corrected at inspection when the director was provided that information from the parent, and filled out that section in the child's enrollment paperwork.
