Caregiver in the infant room was using the changing table to prepare infant's meals.

Two changing mats were observed torn. The child safety alarm in the white van was not working.

Rodent dropping were observed in the cubby area and counter of the Preschool classroom and in the cubby area of the two year old classroom. This was corrected at inspection when the areas were cleaned and sanitized.

A clock and radio sitting on a high shelf had their cords dangling within reach of children sitting underneath the equipment, creating a fall hazard. This was corrected at inspection when the cords were removed.

An infant was sitting in a highchair eating with no safety strap fastened. Note: This was corrected at inspection when infant was remove from the highchair.

During the course of the investigation, it was determined that the operation did not report to Child Care Regulation that a classroom was left unsupervised when a caregiver stepped out of the classroom during naptime.

During the course of the investigation, it was determined that children were left unsupervised while napping in a classroom.

The mulch installed on the big playground is not meeting the 9 inches of loose fill, exposing the concrete around the pole. The mulch installed on the small playground is not meeting the 9 inches of loose fill, as required by CCR.

One staff member had CPR training that was taken online, therefore does not adhere with the guidelines for American Heart Association.

During inspection conducted on 9/25/2025, it was observed that one crib had a cover that was not designated for the size of the mat and not covering the entire mat.

Based on information gathered, the center is not using the playground according to manufacturing instructions. The playground manufacturing indicates for children 2-5 years of age, and child who was injured while using the playground is 23 months old.

Based on information gathered, center staff did not notify parent immediately after child received an injury that caused a friction burn and parent was notified at pick up time.

The director is missing 21.5 hours of annual training.

Two children enrolled are missing signed health statements.

Three out of the ten personnel training files reviewed were missing annual training hours for the last training year.

Three out of the ten children's files evaluated did not have a health statement on file.

Operation did not have documentation on file that parents of children enrolled have been informed of Parent's Rights according to Human Resource Code (HRC) Section 42.04271

Based on the evidence gathered, the fence that encloses the playground had a hole in it large enough for a small child to fit through. This was corrected when the operation repaired the hole.

Three children did not have current immunization records on file.

A child with a diagnosed food allergy did not have a food allergy emergency plan from the doctor on file.

Three caregivers are missing annual training hours for training year 2022-2023.

A caregiver present at the operation did not have an active background check on file. This was corrected when the caregiver left the operation and a BGC request was submitted by the operation for this caregiver.

The director did not have a current CPR/First Aid certification.

One of six children's files did not have a signed medical authorization. This same file did not have an address for the emergency contact.

During inspection, a caregiver was heard telling a child "No,thats disgusting" in an ugly manner. . A child was heard crying immediately after comment was made. When director was asked about it, she said it was due to something the child did.

Three of six caregivers had not competed their required 24 hours of annual training.

Two of six children's files did not include a health care statement.

While reviewing childrens records, one of the four records did not have any emergency contact listed. Operation will obtain information for child's file on Monday 2/28/22 when parents return with child to operation. All have been sick and unable to return to center.

During inspection, the director could not provide a schedule of carbon monoxide texts conducted at operation.

While reviewing records, the director did not provide the current gas inspection report.

While reviewing childrens records, one of the four records did not have any emergency contact listed.

During inspection, the director did not produce daily log of maintenance for playground area, and stated this is not done frequently.
