During the course of the investigation, it was determined that a child was left unattended in the operation's vehicle.

Seven out of nine rooms had three cleaning bottles on the diaper changing table accessible to children. This was corrected when the bottles were stored up high.

Two out of ten children were missing a health statement.

One out of ten children?s records were missing the address of the emergency contact information.

All three vans had expired antiseptic wipes. This was corrected when a new first-aid kit was purchased.

One medication was not in the original container.

Two medications were expired at the time of inspection.

Based on the information gathered a child was left unsupervised on the playground.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022 revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022 as required.
