There were 3 infants that had blankets in the crib. One infant also had a boppy pillow in the crib. The blankets and boppy pillow were reviewed by a caregiver.

There were 2 caregivers that did not have pre-service or orientation on file for review.

There were 3 infants in care that did not have updated infant feeding instructions.

There was one person observed at the operation in a classroom that did not have a background check on file. The person was sent home during the inspection, bringing the deficiency into compliance.

The director had only completed 15 hours of annual training for the previous year.

There were 2 caregivers that did not complete the required annual training for the previous year. One caregiver only completed 12 hours and one caregiver did not have any trainings on file.

The 2 caregivers are drivers for the operation did not have current transportation on file.

During the inspection a caregiver responsible for children under 24 months did not have a current training in the topics sudden infant death syndrome and early childhood brain development.

During the inspection the tires on an piece of active playground equipment was observed with rips exposing the wire.
