One of the outlets in the younger infant room did not have a cover. This was corrected during the inspection when an outlet cover was placed in the outlet.

Two caregivers had a group of seven children ages 4-12 months old, and one of the caregivers stepped out of the room to answer the door, leaving the remaining caregiver out of ratio. Groups of children with a specified age of 12-17 months can only have five children to one caregiver. This was corrected during the inspection when another caregiver came back from break and joined the classroom.

Two infants were observed sleeping with their bottles. This was corrected during the inspection when the bottles were taken away.

An infant was observed sleeping on a boppy pillow on the floor. This was corrected during the inspection when the infant was moved to their crib.

It was found that a caregiver under the age of eighteen was caring for children and counted in a classroom's ratio during the last monitoring inspection.

Out of four staff files reviewed, one staff had not signed a statement that they receivied copies of the personnel and center policies. This was corrected during the inspection when the staff member completed the required statements.

The operation's permit renewal letter was not posted. This was corrected during the inspection when the permit renewal letter was printed and posted.

There were several medications that were expired or no longer needed by the child that were stored in the medication bin. This was corrected during the inspection when the medications were disposed of.

Out of four staff files reviewed, all four were missing training in food allergies and medication administration.

Out of four children's files reviewed, one child's parent had not signed to give the operation permission to obtain emergency medical treatment for the child.

The first aid kit was missing tweezers, gloves, and adhesive tape.

The younger infant room did not have an activity plan posted.

Three medications were not labeled with a child's name. This was corrected during the inspection when the medications were disposed of.

The parent handbook was missing procedures for how to contact the director if parents have concerns or complaints, the benefits of physical activity for children, what kind of bad weather will prevent children from going outside and a plan for children to get their physical activity inside if they can?t go out, information on how parents can access the minimum standards online, and a list of parent?s rights.

Several medications did not have the child's name and date on them.

The infant reports in ProCare did not have the infants' mood and summary of daily activities.

Feeding instructions for infants 0-11 months had not be updated by parents in the last 30 days. New forms were generated for parents to complete at pick up.

Three buckets swings did not have the appropriate use zone required and were not intended for infants and toddlers per manufacturer guidelines. The swings were removed during the inspection.
