It was determined a caregiver did not utilize good judgment when she pulled a chair a child was sitting on and the child fell back.

The last gas inspection report on file was 4/20/2023.

There was a caregiver observed ini a classroom without an active background check. The caregiver was sent home during the inspection.

A child was able to leave the classroom for 16 seconds without any of the caregivers knowing. Once the caregivers became aware of the incident, both caregivers left the children in the classroom without a proper caregiver.

There was sufficient evidence found during the DFPS investigation to show that one child was abused by two employees.

There was sufficient evidence found during the DFPS investigation to show that an employee did not intervene when they saw a child being restrained.

There was sufficient evidence found during the DFPS investigation to show that two employees physically restrained a child on two separate occasions for approximately 8 minutes each.

It was determined a caregiver held a child in care down for 8 minutes.

A caregiver was observed on video inapprorpriately grabbing a child in care.

It was determined a caregiver witnessed a child in distress and did not intervene to deescalate the situation.

A caregiver was heard on video yelling at a child in care.

The operation has not reported the Number of Employees who left in 2024.

There was 1 person observed in a classroom without an active background check on file.

During the investigation operation was found to be deficient in supervision when a child was left on the playground for approximately 3 minutes. Incident was self reported, staff involved files reviewed and they are current on trainings, and supervision was observed without concerns. Operation has met compliance.
