The loose-fill at the bottom of the slide had been displaced.

The infant schedule did not include two outdoor play opportunities or 60 minutes of outside time.

There was no evacuation diagram posted in the pre-k room. This was corrected during the inspection when a copy of the evacuation diagram was posted.

Two caregivers were required to watch the toddler and pre-k classes at naptime, and there was no additional caregiver in the building. This was corrected during the inspection when the director and another caregiver returned to the operation.

There was a can of Lysol spray left on a bookshelf within reach of children. This was corrected during the inspection when the bottle was moved to a shelf out of reach.

A caregiver did not wash an infant's hands before putting the infant down after a diaper change. This was corrected during the inspection when the caregiver was reminded about handwashing and the caregiver lifted the infant up to the sink to wash their hands.

A caregiver stated that they had nine children when there were children in the classroom. This was corrected during the inspection when the caregiver was informed that they had ten children.

A caregiver stepped out of their room to open the front door and left their class briefly unsupervised. Another caregiver was sitting in an office adjacent to the classroom instead of in the classroom, leaving their class unsupervised. These were corrected when both caregivers returned to their respective classrooms.

There was a bottle of Clorox wipes left on a bookshelf in the pre-k class within the reach of children. This was corrected during the inspection when the bottle was moved to a shelf out of reach.

One of the infant caregivers briefly stepped out of the infant room, leaving six infants with one caregiver. This was corrected during the inspection when the caregiver returned to the infant room.

One infant who was still on formula didn't have feeding instructions updated for February or March.

The ramp down to the playground had broken boards on one side of the railing, and there was a loose board on the shed that had exposed nails.

One of the slides on the playground had loose-fill that had compacted and was not six inches deep. There were also two small slides, a seated merry-go-round, and a piece of bouncing equipment that did not have any loose-fill.

The CPSC form was last signed in October 2024.

There were no playground checklists available for review.

The parent handbook was missing parents right information, procedures for accomodating children with special needs, procedures for employee vaccinations, a policy about daily health checks, information on the benefits of physical activity, a plan for indoor physical activity to occur when children can't go outside, information on how parents can review minimum standards online, and policies for applying insect repellant and sunscreen.

The operation did not have a list of employees posted where parents could view it.

The infants in the infant room were watching television. This was corrected during the inspection when the television was turned off.

A 5-month-old infant was sleeping in the crib with a burp cloth. This was corrected during the inspection when the burp cloth was removed from the crib.

There was TV playing in the infant room. This was corrected during the inspection when the caregivers were informed that screen time can't be used for children under two and the TV was turned off.

The infant caregivers and the director did not have training in SIDS, shaken baby, and early childhood brain development for 2024.

There were two caregivers with a group of 13 children whose specified age was 18-23 months. One of the caregivers briefly left the classroom, leaving the group out of ratio. This was corrected during the inspection when the caregiver returned to the classroom.

Some antibiotic and anti-itch cream in the first aid kit had recently expired. This was corrected during the inspection when the creams were disposed of.

The operation did not have a signed children's product certification form verifying that the director had reviewed the children's product recall list. This was corrected during the inspection when the director confirmed that she is subscribed to the recall emails and signed a certification form.

Of the four infants who were still on breastmilk or formula, two did not have feeding instructions on file and one had feeding instructions that were last updated in August.

The first aid kit was missing tweezers, gloves, and a first aid guide.

One infant's written feeding instructions had not been updated since March.

There were no playground maintenance checklists available for review. This was corrected during the inspection when the director conducted a check of the playground and completed a playground maintenance checklist.

Out of the four children's records reviewed, two were missing health statements, two needed updated immunization records, and one was missing hearing and vision screening results.

There was a slide under five feet tall with no loose-fill in the use zones.

A child was sent to the bathroom without being accompanied by a caregiver. This was corrected during the inspection when the child returned to the classroom and the caregiver was informed that children cannot be sent to the bathroom alone.

There were a bottle of Clorox wipes sitting on a shelf within the reach of children. This was corrected during the inspection when the wipes were moved to a shelf out of the reach of children.

Out of the four staff files reviewed, four were missing proof of background check requests, one was missing a photo ID, and one was missing a copy of her GED.

Uncovered outlets were observed in the pre-k room. This was corrected during the inspection when outlet covers were placed on the outlets.

Out of the four staff files reviewed, one caregiver was missing 15 out of 24 hours of annual training, one was missing training in preventing emergencies due to allergies, one was missing training in preventing abuse and neglect, and one was missing training in preventing abuse and neglect, SIDS/Shaken Baby Syndrome, early childhood brain development, emergency preparedness, preventing emergencies due to allergies, building and physical premises safety, and handling hazardous materials.

During the inspection there was no documentation of a infant caregiver having the required pre-service trainings for caring for children under 24 months.

During the inspection the last fire drill was documented in March 2021. This was corrected during the inspection by a fire drill being condcuted.

A slide and a rocking plane was observed on the playground without loose fill.
