One child in care did not have a copy of their immunization records on fie for review.

2 out of 6 personnel files reviewed did not have Form 2912 on file for review.

Screen time was observed being utilized for children under two years old. The caregiver turned off the tablet once she was made aware of the standard.

It was determined proper supervision was not maintained during bathroom breaks, as children have been sent to the bathroom without a caregiver present.

A bottle warmer was observed on video, accessible to an infant in their crib. The infant received an injury when they were able to grab a hold of the bottle warmer.

It was determined the caregivers did not utilize good judgement when they placed cribs too close to a bottle warmer, which caused a child to receive an injury that required medical attention.

An infant was observed in a bucket seat and was observed to not have been fastened. The caregiver fastened the infant during the inspection.

It was determined proper supervision was not maintained in a classroom, where several children were bit multiple times, several times a week.

There were 4 children in care with food allergies and the operation did not have a food allergy plan on file for any of the children.

An infant was observed asleep in a restrictive device with a blanket on the infant's body. The caregiver informed that the infant has been asleep in the swing for approximately 18 minutes. The caregiver removed the infant and placed the infant in a crib.

There were 5 infants that did not have updated infant feeding instructions on file for review. Three of the parents were able to update their infant feeing instructions during the inspection.

The operation did not have an incident report for a situation that placed children at risk.

It was determined proper supervision was not maintained during the time of the incident.

It was determined an incident occurred at the operation that placed children at risk and the operation did not report the incident to Child Care Regulation. The required notifications were reviewed with the Director.

The operation did not have a current fire inspection on fire. The operation's last fire inspection was completed in November 2023.

A caregiver was observed in a classroom without an active background check on file. The caregiver was replaced by a caregiver with an active background check.

It was determined there was an incident where the caregivers did not share information with one another which cause the child to appear lost when the parent came to pick up the child.

It was determined during the time of the incident the caregivers were observed cleaning and not actively supervising the children in care.

It was determined proper supervision was not maintained during the time of the incident.

It was determined a caregiver inappropriately grabbed a child's arm and caused an injury that required medical attention.

There were 5 infant feeding instructions that were not updated for the month of May.

It was determined an child received an accidental injury on 3/28/23 and required medical attention. The incident was not reported to Child Care Regulation until 4/12/23.

During the inspection it was determined a Toddler classroom was changed to an infant classroom and Child Care Regulation was not notified of the change.

It was determined children were being placed in buggies while caregivers were preforming janitorial duties at the end of the day.
