The permit renewal letter was not posted. This was corrected during the inspection when the letter was printed out and posted.

A caregiver stated that they had 13 children in their group when they were responsible for two classes combined and had 20 children in their group.

There was a three-year-old infant/toddler room where the youngest child was sixteen months old. This was corrected when the three-year-old was sent to the pre-k room. The next oldest child in the infant/toddler room was 25 months old.

A caregiver changed a child's diaper on the floor where children were playing.

A child had their diaper changed and did not wash hands afterwards. This was corrected during the inspection when the caregiver was reminded about handwashing and washed the child's hands.

There was a bottle of Clorox wipes on a bookshelf within reach of children. This was corrected during the inspection when the caregiver moved them to a higher shelf out of reach.

The first aid kit at the new building was missing a thermometer and a guide to first aid.

There was a slide structure on the playground that did not have any loose-fill.

Infant reports were not being completed.

The toddlers did not have two outdoor opportunities scheduled and the pre-k class did not have 90 minutes of physical activities scheduled.

The operation changed the room that the infants were in to a room that was not previously licensed for infants and stopped using their second infant room without notifying Child Care Regulation. This was corrected during the inspection when the director informed CCR that the infants were now in the old toddler room.

The operation has a chicken coop on the playground, and the children are able to touch the chickens through the gate of the coop which is made of a steel wire fence with holes large enough to allow contact.

One caregiver was missing training in preventing the spread of communicable diseases.

The first aid kit was missing a guide to first aid, antiseptic solution or wipes, and gloves.

During the investigation, there was sufficient evidence to determine that two children were able to leave their classroom unsupervised while their caregiver was distracted.

Two outlets in the toddler room were missing covers. This was corrected during the inspection when outlet covers were placed in the outlets.

A caregiver stated they had ten children in care when they had nine children.

All slides that are five feet or smaller must have a minimum of six inches of loose-fill in the use zones. The use zones for these slides must extend six feet out from the exit, six feet from the access point where children climb up, and six feet to each side. Having loose-fill in these areas will help prevent injuries, as it provides a shock-aborbing surface in case of a fall.

The operation did not have a menu for May posted. This was corrected during the inspection when a menu was created and posted.

This operation did not have a current list of employees posted.

The operation did not conduct a fire drill in March. This was corrected during the inspection when a fire drill was conducted and the children were able to exit within three minutes.

One of the pre-k classes only had 60 minutes of physical activity on the schedule and some of the toddler classes only had 45 minutes of physical activity scheduled.

One infant who was still on formula did not have updated feeding instructions. This was corrected during the inspection when the parent signed and dated the feeding instructions to indicate that they had not changed.

The operation did not have a children's product certification form available for review.

A review of the Employees Who Left Employment in 2024 data revealed the center did not report the number of employees who left employment in 2024 by January 15, 2025, as required.

Out of eight caregivers' files reviewed, one did not have training in the following required topics: abuse and neglect, communicable diseases, food allergies, and handling hazardous materials. This was corrected during the inspection as the caregiver is currently on leave.

Out of eight children's records reviewed, one child was missing an admission date, one was missing an an updated immunization record, and one was missing a physician's health statement.

Two infants who were not on table food yet had feeding instructions that were not updated monthly.

An electrical outlet in one of the infant rooms did not have a childproof cover. This was corrected during inspection when the director put a childproof cover on the outlet.

The parent handbook did not have the criteria for extreme weather that will prohibit children from going outside to play.

There was a bottle of cleaning solution in an unlocked cabinet where children could access it. This was corrected during the inspection when the caregiver moved the bottle to a drawer out of the reach of the children.

The infant daily reports did not have sections for each infant's mood and a summary of daily activities.

A caregiver in care did not have current CPR/First Aid certification on file. The certification expired in May 2023.

The children in the Toddler classroom were observed being changed on the floor where the children were observed playing. This was corrected when the caregiver sanitized the area and moved the diaper changing area.

During the inspection a 7 month old was observed sleeping on a sheet that was loose and could cause an entanglement. THis was corrected the caregivers pulling the sheet snugly.

Three caregivers observed with infants under 17 months old did not have proof of the required orientation and annual infant trainings.

During the inspection a 3 month old was observed in a swing sleep in a swing swaddled with a blanket. This was corrected by the blanket and the infant being removed.

During the inspection infants bottles were observed not labeled. This was corrected during the inspection by the caregiver labeling the bottles.

A caregiver responsible for children during the inspection did not have documented proof of pre-service training or pre-service exemption.

During the inspection two caregivers CPR certificates were evaluated and found to be taken online only.

During the inspection observed children in care enter the playground without the Designee conducting a daily safety check first. The daily grounds checklist was documented 2 out of 5 days. This was corrected during the inspection by the Designee checking the playground for hazards and documenting the check.

The last fire drill documented fire drill was completed in October 2021. This was corrected during the inspection by the operation completing a fire drill and exiting within the required time frame.

During the inspection the personnel files were not available for review.

During the inspection one caregiver was on the playground in the care of eighteen ages 20 months to 3 years old. This was corrected by a second caregiver returning to the playground.

During the inspection a recall rock n play was observed in the infant classroom. This was corrected during the inspection by the recall item being removed from the classroom and placed in the office
