There was a bottle of Clorox wipes on the floor within reach of children. This was corrected during the inspection when the wipes were moved.

The smoke detector in the kitchen, which is a room used by children, was beeping.

Out of four children's files reviewed, one child was missing an emergency contact, two were missing a parent's signature agreeing to the parent handbook, one was missing a health statement, two were missing immunization records, one was missing a medication administration log, and all four were missing sign-in and -out logs.

Two of the smoke detectors in the home were beeping and needed new batteries. One of the smoke detector's batteries was changed during the inspection.

The permit renewal letter and employee list were not posted.

The CPSC form was last signed 5/15/24. This was corrected during the inspection when the caregiver signed the form verifying that she has reviewed the recall list.

One household member who was not counted in ratio was missing documentation of orientation.

The dates of the most recent fire drill and tests for fire extinguishers, carbon monoxide detectors, and smoke detectors were not recorded. This was corrected during the inspection when the drills and tests were recorded.

The first aid kit did not have a guide to first aid.

The home's most recent licensing report was not posted.

The children's product recall form had not been signed since 2022. This was corrected during the inspection when the primary caregiver signed a new form verifying that she reviewed the recall list.

The primary caregiver did not have training certificates available for review for training in SIDS and Shaken Baby Syndrome, early childhood brain development, abuse and neglect, emergency preparedness, communicable diseases, administering medication, preventing emergencies due to allergic reactions, understanding building and physical premises safety, or handling hazardous materials.

A cat in the home did not have any vaccination records available for review.

Out of four children's records reviewed, two did not have the admission date and four needed updated immunization records.

During the inspection the provider did not have documentation of current vaccinations for pets in the home.

During the inspection the operational policies did not have a policy on the promotion of indoor/outdoor physical activities.

During the inspection it was found that the primary caregiver CPR/First Aid certificate expired in December 2020.
